**Job Description:**
The Medical Records/Health Information clerk organizes and evaluates medical records for completeness and accuracy according to facility procedures, and may be assigned to code diagnoses and procedures.
**Essential Function:**
• Checks the dismissal charts for completeness and reports chart record deficiencies.
• Performs release of information according to department procedures. Notifies supervisor prior to releasing medical information to outside sources, i.e., attorney requests,
And notifies physician offices as requested.
• Notifies lead clerks and/or supervisor of any discrepancies or problems with the records or recordkeeping process.
• Works with department contacts and other staff to route patient charts to proper locations.
• Monitors patient records and/or merges the index as necessary.
• Communicates and protects medical records information in accordance with established procedures.
• May have access to personal health information (PHI) necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization/department.
• Performs clerical responsibilities within the medical records department, as assigned.
• Process physician orders & track information.
